Patient Benefits - CCAMD’s patient membership programs are designed to be flexible and to provide more choices that align the membership plan with the patient’s medical status and with the patient’s household income. The physician will be able to provide significantly more time focused on improving health status and preventive care. CCAMD membership plans offer:

  

  • A low-cost basic membership that enables patients same day/next day appointments, communications by phone and e-mail Monday-Friday 8-6 office hours, enhanced lab capabilities that include web and tele support with coaches and dieticians and - 
  • A socio-economic price sensitive premium membership model, with same day appointments, 24/7/365 access by phone and e-mail and other enhanced benefits for patients.


With greater access and more time with their physician, CCAMD members will benefit from a more meaningful pro-health relationship providing the basic foundation for the medical home. CCAMD will be providing additional value added services predominately through complementary strategic partnerships that will include benefits such as access via telehealth and patient portals to coaching, dietetic and other wellness support services.
